    Form 498 Status Search Tool

    Enter a partial or full Applicant Name and/or select a State. Click "Search" to see the results.

    Important Information

    The Form 498 Status Search Tool allows users to check the status of their organization’s Form 498. To further filter your search results, click on Table Options link at the bottom of the table and select Show Filters. A row will be added at the top of the table where you can type additional search terms.
    E-rate Manager® account holders can download the search results by clicking the Download link, also located at the bottom of the search results table.
